Operations Management Analyst

Essential Functions/Areas of Accountability

  • Design/conduct organizational analysis of business operations/create operational systems, procedures, manuals/policies to increase operational productivity & efficiency.
  • Research, cost justify, test/implement productivity improvements, compliance initiatives.
  • Quality assurance measures and system upgrades for application to healthcare staffing agency operations, including development/coordination of recruitment/hiring programs.

Case Managers

Provides direct care and case management for a team of Active Home Health clients in accordance with the state Nurse Practice Act and Active Home Health policy.

Directs: Assigned team members of RNs, LPNs, Home Health Aides, and Personal Care Assistants.

Qualifications

Minimum of one (1) year of Case Management experience. Home care experience preferred.

  • Ability to supervise and direct members of client care team.
  • Demonstrated strong and effective written and verbal communication skills and good interpersonal skills.
  • Ability to deal tactfully with clients and the community.
  • Demonstrated organizational and time management skills.
  • Knowledge of home health regulatory and reimbursement requirements.
  • Licensed driver with automobile insured in accordance with state and/or Active Home Health requirements and in good repair.

Essential Functions/Areas of Accountability

  • Reviews client referral information and responds to requests and inquiries as appropriate.
  • Performs initial and ongoing client assessments based on Active Home Health policy and standards of practice to ensure effective and appropriate home care services.
  • Directs, plans, and initiates appropriate action independently and responsively in
    home care situations.
  • Manages/supervises a team of RNs, LPNs, Home Health Aides, and Personal Care Aides to provide effective and quality home care services.
  • Participates in human resource management to achieve quality service delivery and positive employee relations.
  • Promotes personal safety and a safe environment for clients and coworkers.
  • Performs job duties in accordance with Active Home Health policies, procedures, and professional and community standards.
  • Performs other job duties as assigned.

Registered Nurse

Provides services to clients in accordance with the State Nurse Practice Act, Active Home Health policy, and accepted professional standards of practice.

The Registered Nurse provides nursing services utilizing a comprehensive base of nursing theory and nursing process, and communicates/documents observations and assessments.

Qualifications

  • Graduate of an accredited school of professional nursing. BSN, PHN preferred.
  • Current license as a Registered Nurse in the state of Florida.
  • Minimum of one (1) year experience in an acute care setting or equivalent experience.
  • Licensed driver with automobile insured in accordance with state and/or Active Home Health requirements and in good repair. Good driving record.
  • Current CPR certification.
  • Strong communication and interpersonal skills.

Essential Functions/Areas of Accountability

  • Assumes responsibility and accountability for the practice of professional nursing in accordance with the State Nurse Practice Act and standards for home health nursing.
    • Performs comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
    • Completes comprehensive assessments including OASIS data set within forty-eight (48) hours of referral for speech therapy unless other arrangements are made.
    • Develops and/or follows an individualized Plan of Care that incorporates analysis of data and current scientific findings.
    • Provides skilled interventions with a focus for achieving realistic client outcomes within a specified time period.
    • Reports changes and information necessary to modify and update the care plan to reflect progress towards goals (outcomes).
  • Consistently demonstrates competency with technical nursing skills according to personal and legal scope of practice:
  • Practices nursing with respect for individual, cultural, and spiritual differences.
  • Communicates effectively and in accordance with Active Home Health requirements.
  • Initiates actions to improve the quality of patient care. Takes appropriate actions independently and responsively in home care situations. Initiates referrals as needed to other health team members when delegated by the Case Manager.
  • Maintains confidentiality in all aspects of the job.
  • Promotes personal safety and a safe environment for clients and coworkers.
  • Participates in ongoing, professional self-development.
  • Supervises applicable home care team members as delegated by RN Case Manager.
  • Performs other duties as assigned.

Licensed Practical Nurse

Performs skilled nursing functions in accordance with the federal, state, and local laws and within the guidelines of his/her professional organization and Active Home Health.

All functions shall be performed in accordance with the established policies and practices and state Nurse Practice Act. The LPN/LVN delivers care to clients as delegated by the RN Case Manager.

Qualifications

  • Graduate of an accredited school of professional nursing.
  • Current license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse in the State of Florida.
  • Minimum of one (1) year experience in an acute care setting or equivalent experience.
  • CPR certified.
  • Demonstrated written, verbal, and interpersonal communication skills.
  • Licensed driver with automobile insured in accordance with state and/or Active Home Health requirements and in good repair. Has a good driving record.

Essential Functions/Areas of Accountability

  • Provides professional nursing care as defined in the nurse practice act and under the direction of a Registered Nurse. Communicates appropriate status/condition changes to the RN per Active Home Health guidelines.
  • Promotes personal safety and a safe environment for clients and coworkers.
  • Performs job in compliance with Active Home Health policies and procedures and professional and community standards.
  • Participates in coordination of home care services, attends meetings as required, and follows reporting guidelines.
  • Performs other related duties and responsibilities as deemed necessary.

Marketing Director

Responsible for managing marketing program and supervising members of the marketing team.. Establishes and maintains relationships with customers and referral sources.

Responds to requests and concerns. Negotiates contracts with third party payers, government agencies and other entities requesting services.

Qualifications

  • Bachelor’s degree in business or related field.
  • Previous experience in health care management or marketing, preferably in home health care.
  • Demonstrated knowledge of regulatory requirements and restrictions in home health services.
  • Demonstrated supervisory and negotiation skills.
  • Excellent verbal, written communication skills.
  • Demonstrated organizational skills, flexibility, assertiveness, and team orientation.

Essential Functions/Areas of Accountability

  • Directs marketing operations and establishes and implements marketing initiatives.
  • Negotiates contracts with managed care companies, government agencies and other payers that ensure optimal reimbursement.
  • Establishes systems for consistent market assessment and develops a marketing plan designed to meet Active Home Health goals.
  • Establishes and maintains mutually beneficial working relationships with current and potential referral sources and payers.
  • Negotiates pricing with insurance case managers and other payers within acceptable standards of practice.
  • Participates in budget development for the department and establishes allocations. Monitors use of resources within budget limitations.
  • Recruits, hires, orients and manages members of the marketing team.
  • Participates with management team in strategic planning activities, identifying opportunities to improve services and better meet customer needs.
  • Maintains comprehensive knowledge of Active Home Health’s markets, key referral sources, and position in the market related to competitors.
  • Develops and maintains information on available community resources, and assists customers and staff in accessing services or information.
  • Participates in annual Active Home Health evaluation in areas of marketing, program development, growth and expectations.
  • Prepares reports of marketing activities and effectiveness.
  • Works collaboratively with clinical staff to assure consistent message to the customers and the community.
  • Other activities as directed.
